TheAPlan Profile Survey


The superstar of the TheAPlan system is the Profile database, comprised entirely of health-conscious people, all of whom have granted their permission to be contacted about their health concerns and/or needs.

Each and every Profile has recently completed the online health survey you see below. Notice that the survey is extensive and provides tremendous insights about their health needs and interests before you even contact them!

The survey must be completed entirely (i.e. no partially completed surveys are included in the database). Once you have enrolled in the system, you will be authorized to access the Profile database. You may want to review the database manually to look at how the Profiles completed individual, or you can use advanced search filters to find the Profiles who match your exact interests.

*The survey below is what all profiles must complete in order to be added to the profile database. The survey must be completed entirely, (no partially completed surveys) otherwise the profile will be rejected and not be added to the database. Once you have enrolled in the system, you will be granted access to the database. You can search the entire database using advanced filtering options to find the profiles that match your exact interests.
